Suffolk Sport invites you to nominate individuals, Clubs, or Teams that have achieved immense success.

Recognition Scheme Criteria

Individual Performers -
A performer, who has made great progress, achieved outstanding success on the national or international stage.
A performer that has been recently selected to represent their Country.

Club Achievements  -
The Club that has recently achieved Clubmark or equivalent NGB accreditation i.e. Charter Standard Community, Swim21 etc.

Team Achievements -
A team that has achieved immense success at national standard or above.

Exceptional Achievers -
An individual/group that have gone above and beyond the call of duty.
An individual or group that have made an outstanding contribution to sport in Suffolk, whether this be through dedicating a lifetime to sport in the County or achieving momentous but specific success. (i.e. securing significant funding for a community club or project)

• Details of why the individual/group is eligible to receive recognition.
• Name and address of candidate.

Once Suffolk Sport have assessed your nomination, you will be informed whether the candidate will/will not receive a letter of recognition.
